Chronic Disease Management
In Internal medicine, we see many common chronic conditions such as asthma, diabetes, high cholesterol, heart disease, arthritis, and many others. If you suffer from, or are diagnosed with any chronic disease, our providers will partner with you to develop a comprehensive plan and help keep your health on track. Our providers also work with your other health care specialist to ensure that your care is coordinated and effective.
Wellness and Prevention
The best way to prevent illness and injury is to keep you and your body in good health. As experts in adult health, our providers will help you develop a plan for health and wellness through annual preventative exams (commonly referred to a physical exams). The annual preventative exam consists of an open discussion of your overall health history and review of each of your body systems to ensure that everything is working properly. We will verify all medications to ensure there are no adverse interactions, verify that your immunizations are up-to-date and do routine blood testing. Finally, based on your age, sex and the CDC guidelines, you may be referred to outpatient imaging for testing such as bone density, mammogram, colonoscopy and/or prostate exam.
